A few years ago when I was teaching first grade, I was given a teacher favorite things form from a student to fill out. It asked simple questions like my favorite Starbucks drink and what supplies I always needed for the classroom.
It really touched me and meant a lot that this family wanted to get to know me.
Sure enough, throughout the year this family was constantly doing little things that made me feel so special. Sometimes it was a cold drink at dismissal and other times it was sending in my favorite snack on a random Tuesday.
Now that I am a parent, I am always looking for ways to show my love and appreciation for my children’s teachers. So, I put together this Teacher Favorites printable to share with you too!

How to Use the Teacher Favorites Printable
Using the teachers’ favorite things form is simple. First, download and print off the form for free by clicking the picture above. Both a teacher and staff member can be found in the download to include everyone in the school.
Next, you can write a little personalized note on the form or attach it with a post-it note. It should say something along these lines:
Child’s name is so excited to be part of your class this year! We would like to know some of your favorite things so we can show you our love and appreciation throughout this school year. Please send this back home at your earliest convenience.
Be sure to include your child’s name so they know who sent it too!
Next, give the printable to your child’s teacher preferably at the beginning of the year. If your child has multiple teachers and you want to ensure that each teacher gets a form; write the teacher’s name in the blank.

Helpful Tips for Using the Teacher Favorite Things Form
- After the teacher completes the teacher’s favorite things form, snap a picture of it on your phone. That way you have it when you are out and about shopping or if another parent asks for it.
- Don’t forget about special teachers and other special school staff members!
- Add important dates to your calendar and plan ahead for when you want to treat your child’s teacher.
- If you are a teacher, you can display this form on Meet the Teacher Night or even recommend that your PTA collect for each teacher and keep them on hand in the office.
